Do you need access to my Google Ads account?

No. Your publicly reachable sitemap is enough for the feed. You push the finished page feed to Google Ads yourself.

Does it work with messy URLs?

Yes. We read titles, descriptions and structured data, not just the URL path. Descriptive URLs help, but they are not a requirement.
